I have been informed that my adopted dog has tested HW+ (Heart Worm positive). I have been provided with the 2021-2022 GPPR Heartworm Treatment & Prevention Protocol document and understand the reading including the slow kill and fast kill methods of treatment. Initial here as informed and understand:
*
I have consulted with my veterinarian on both treatment methods and have done additional research. Initial here you have consulted: *
I agree to stay in contact with Great Pyrs & Paws Rescue & understand the Medical Coordinator team will reach out periodically to check on my adopted dog's HW status. Initial here to agree & understand: *
I understand I will receive 3 doses of Advantage Multi Flea, Tick, and Heartworm monthly topical solution (or a similar brand) and Doxycycline if my dog has not completed this treatment prior to adoption. I understand it is my responsibility to administer the medications on time and if I have questions I will reach out to our medical coordinator team at greatpyrsandpaws@gmail.com Initial here you understand: *
I understand completely the benefits & risks of the slow kill and fast kill methods of treatment and that I will consult with my veterinarian after the 3 months of Advantage Multi are depleted for further treatment. I also understand an additional test to determine worm load might be beneficial in deciding the next treatment route. Initial here you understand: *
I understand that due to physiological differences of the Great Pyrenees breed, advocates state these dogs have slower metabolism and higher negative physical reactions to the medications in the "fast kill" treatments. Because of this, the Great Pyrs & Paws Rescue highly recommends that adopters adopting HW+ dogs through this rescue consider the "slow kill treatment" which could take up to 18 months. Initial here you understand: *
I understand that once my dog tests clear of heartworms, that he/she needs to be given heartworm preventative (such as heartguard) monthly for another year or as recommended by my veterinarian to prevent reinfection. Initial here you understand: *
THE undersigned PARTY/PARTIES fully understand and agree to be bound to this agreement. By typing my name(s) in the box below, I AGREE TO BE BOUND TO THIS AGREEMENT. All surrendering parties must sign. *
