I/we agree to release GPPR from any and all claims, known and unknown, now or hereafter, arising in connection with the dog while under the direct care of the surrendering family. I/we understand according to the OSAP program I/we are required to have the dog up to date on all vet care including spay/neuter, vaccines, preventative, and routine medical care. I/we have provided the rescue with these records to enter into the OSAP program. I/we understand I/we are responsible for any necessary vet care while the dog is in my/our care. I/we have disclosed any challenging behaviors to the rescue and have had my/our dog assessed by a behaviorist and/or trainer. I/We have provided this to the rescue and will disclose these behaviors and provide the behaviorist/trainer's information to any potential adopter. I/we give permission for the rescue to list the dog on adoption software Rescue Groups which posts the dog to Petfinders and Adopt a Pet. I also understand the dog will be featured on the rescue's social media Facebook and Instagram. Initial to give permission: *
I/we understand the rescue will process all applications for the dog through the rescue's software platform Rescue Groups. Either the Foster Coordinators or Adoption Counselors will give the me/us the information on approved families. I/we will then make every effort to contact the approved family within 48 hours and arrange for them to meet the dog. Initial here to agree: *
In conjunction with the Foster Coordinator and Adoption Counselors, I/we understand I/we have a say in whether the approved family is the right home for the dog. I/we do not have to send a dog home with a family I/we feel is unsuitable. You will contact either the Foster Coordinator or Adoption Counselor to explain why you feel the match is unsuitable. I/we understand once the adoption donation fee has been paid by the new family and they take the dog home I/we cannot request to take the dog back.
